　一次性下肢静脈瘤の不全穿通枝に対する細径内視鏡を用いた内視鏡下穿通枝切離術（subfascial endoscopic perforator surgery：SEPS）について解説する．不全穿通枝の検索は，上行性下肢静脈造影およびduplexスキャンで行った。SEPSの手技は，下腿内側からの一点穿刺で行い，内視鏡と同軸で操作が行えるシステムでpotassium-titanyl-phosphate（KTP）レーザーを使用して施行した．神経痛などの合併症はみられず，症状は著明に改善し，現在までに静脈瘤の再発は認めていない．SEPSによって低侵襲で確実な不全穿通枝の遮断が可能となり，さらに，軟性鏡の応用で短時間で容易な手術手技となった．美容的観点からも細径内視鏡によるSEPSは有用である．

We have performed subfascial endoscopic perforator surgery (SEPS) by one puncture method using small caliber fiberscope system for patients with primary varicose vein in the great saphenous vein. Incompetent per-forators were examined by ascending phlebography and duplex scanning. In performing SEPS, we have devel-oped our own system aiming at minimally invasive surgery. SEPS was performed using KTP Laser. In our system, the procedure can be done coaxially with a 3mm endoscope or 2.7mm flexible fiberscope by 1-point puncture on the medial aspect of the lower leg.
