内視鏡下静脈手術で用いるサージカルデバイスの適切な使用法

Appropriate use and maneuver of surgical devices for the endoscopic venous operations:Technical points of performing SEPS

　内視鏡下筋膜下穿通枝切離術（subfascial endoscopic perforator surgery：SEPS）は本邦でも1994年から開始され，その後各施設で工夫がなされたことにより，ほぼ安定した手術成績が報告されるようになった．うっ滞性皮膚潰瘍を有するような重症静脈瘤症例における下腿の不全穿通枝（incompetent perforating vein：IPV）に対して施行するSEPSで用いる各種サージカルデバイスの適切な使用法と要点について，手技の手順にしたがって紹介する．すなわち（1）手術の準備，（2）筋膜下層の組織剥離，working spaceの作製と確保，（3）IPVの確認と穿通枝切離に用いる器具の特長，（4）感染，出血，熱損傷，神経損傷などの合併症の回避について解説する．

By the introduction of minimally invasive technique; subfascial endoscopic perforator surgery (SEPS), sur-geons have become increasingly attracted to the use of SEPS for chronic venous insufficiency accompanied with such as stasis ulcer. The safety, feasibility, and efficacy of SEPS have been widely demonstrated. In this report, we showed appropriate use of various surgical devices for the following procedures; approaching the subfascial space, dissection of the subfascial tissues, securing the working space, and isolation and division of incompetent perforating veins (IPVs).
