CO2送気下内視鏡的筋膜下穿通枝切離術

Subfascial Endoscopic Perforator Surgery under Carbon Dioxide Insufflation

　CO₂送気下subfascial endoscopic perforator surgery（SEPS）を行う際のポイントと，われわれの手技について述べた．CO₂送気下SEPSは，不全穿通枝の切離が主目的となる症例が対象となり，CO₂ガス漏れが起きやすいのでトロッカーの選択が重要である，われわれは皮下内視鏡手術用トロッカーとして，独自に開発したソフトトロッカーを使用している．筋膜下の？離は内視鏡下に行い，バルーンを併用する．穿通枝の切離には超音波凝固切開装置を使用する．CO₂送気下SEPSは視野および鉗子の操作性は良好であるが，侵襲は決して軽微なわけではなく，よく適応を検討して行う必要がある．

In this report, the essential points of subfascial endoscopic perforator surgery (SEPS) under CO₂ insufflation and our procedure of SEPS were shown. The patients with chronic venous insufficiency, which is mainly caused by incompetent perforators, were assigned for SEPS under CO₂ insufflation. It is important to select the appro-priate trocar because gas leakage sometimes occurs during subcutaneous use of the conventional laparoscopic trocar.
