腹腔鏡補助下幽門側胃切除術における新型腹腔鏡用腸鉗子を用いた体外からの体内吻合

Intracorporeal Anastomosis Performed Extracorporeally using New Type of Forceps in Laparoscopically Assisted Distal Gastrectomy

　腹腔鏡補助下胃切除術後の再建は，一般的には，胃を摘出した上腹部の創を利用して，体外で行われている．体外吻合では，創が小さいと十二指腸の挙上が困難となる場合がある．一方，体内吻合においては，習熟した術者でもかなりの時間を要する．そこで，消化管の吻合部は体内におき，小開腹創を介して体外から通常の器具で吻合を可能にする新しい腸鉗子を考案した．この鉗子は，腹腔鏡ポートから挿入でき，吻合する消化管同士を体内で平行に保ち，かつ，顎の部分が平行に閉じるように，また，咬合部は2列の鋸歯状に設計した．ブタを用い，幽門側胃切除術を行ったのち，本鉗子を用いて体外から体内で胃十二指腸吻合を行った．この方法は直視下のため，縫合・結紮が容易であった．

During laparoscopically assisted distal gastrectomy (LADG), many surgeons perform gastroduodenostomy extracorporeally. A 5-6cm incision is made in the upper epigastrium, the stomach is removed and anastomosis is done extracorporeally. Totally laparoscopic anastomosis without circular stapler is also a good procedure, but only the most skilled laparoscopoic surgeons can do it. In addition, many talented surgeons can perform open distal gastrectomy with a 10cm incision, therefore they feel LADG has no big advantage.
