腹腔鏡補助下幽門側胃切除術の定型化にむけて―総肝動脈の門脈背側走行症例を経験して―

Early gastric cancer with vascular anomaly experienced during development of a standard laparoscopy-assisted distal gastrectomy technique

◆要旨：筆者らは，腹腔鏡補助下胃切除術における術式の定型化とランドマーク設定を行っている．今回，総肝動脈が門脈背側を走行する症例を術中に把握し，安全に手術を遂行できたので報告する．胃角部前壁の早期胃癌に対して腹腔鏡補助下幽門側胃切除術を施行した．No．8aリンパ節郭清の際，膵上縁に総肝動脈が認められずAdachiⅥ型の血管走行バリエーションを考慮したが，ランドマークを順次同定し手術を進めることで，安全に手術を遂行することができた．胃癌に対する胃切除術に限らず，腹腔鏡下手術の場合，どのようなバリエーションに対しても解剖誤認を起こさないために，術式の定型化とランドマークの適切な設定が重要である．

　Currently, we are developing a standard technique for laparoscopy-assisted gastrectomy. In the lymph node dissection of laparoscopy-assisted gastrectomy, caution should be exercised in patients with vascular anomaly. We report a case with vascular anomaly of the common hepatic artery which runs on the back side of the portal vein. A 65-year-old man was referred to us for treatment of early gastric cancer, and we performed laparoscopy-assisted distal gastrectomy. In the operation, we found the vascular anomaly, and the operation was performed safely by our stylized technique. This experience teaches us a renewed recognition of the importance of the stylized technique for laparoscopy-assisted gastrectomy.
