 Currently, we are developing a standard technique for laparoscopy-assisted gastrectomy. In the lymph node dissection of laparoscopy-assisted gastrectomy, caution should be exercised in patients with vascular anomaly. We report a case with vascular anomaly of the common hepatic artery which runs on the back side of the portal vein. A 65-year-old man was referred to us for treatment of early gastric cancer, and we performed laparoscopy-assisted distal gastrectomy. In the operation, we found the vascular anomaly, and the operation was performed safely by our stylized technique. This experience teaches us a renewed recognition of the importance of the stylized technique for laparoscopy-assisted gastrectomy.
