◆要旨：胃癌に対する腹腔鏡下幽門側胃切除術は安全性と根治性を損なわない低侵襲治療として普及してきている．しかし，術後合併症の半数以上は吻合に関連するものであり，幽門側胃切除後の再建は，安全，確実に行うことが必要である．そのためには，再建法は症例に応じて選択し，吻合は簡便で確実な器械を用いて，良好な視野が確保される体内吻合で行うことが望ましい．当施設ではBillroth I法（デルタ吻合），逆蠕動あるいは順蠕動Roux-Y法を体内器械吻合で行っているので，その手技を紹介する．体内器械吻合は有用な方法であり，腹腔鏡下幽門側胃切除術を行うにあたってはその技術を習得することが必須である．
Laparoscopic gastrectomy for gastric cancer has become popular as a less invasive and safe procedure without diminishing the radical quality of the surgery. Therefore, in the laparoscopic distal gastrectomy, it is extremely important to make the reconstruction without any complications. The reconstruction method should be selected based on the situation of the patient. Because stapling devices make the procedures easier and safer, and intracorporeal procedures provide a good field of view, the intracorporeal anastomosis using linear staplers is very useful after distal gastrectomy. We demonstrate the techniques of Billroth I method, and antiperistaltic or isoperistaltic Roux-Y method. The intracorporeal anastomosis using stapling devices has an advantage in the laparoscopic distal gastrectomy, and the surgical techniques should be learned.
