Robotics in Gastrointestinal Surgery

　2000年3月からIntuitive Surgical社の内視鏡下手術支援システム，da Vinci^TMを臨床へ導入し，食道胃逆流症に対するNissen手術および食道アカラシアに対するHeller and Dor手術，胆嚢摘出術を中心に，多様な術式において用いてきた．癌治療への応用として，総胆管空腸吻合，経胸的肝部分切除術においても利用した．現時点では，da Vinci^TM導入による新たな臨床的利点は顕著ではないが，本システムによるscaling機能やfiltering機能を備えた6自由度の鉗子操作と高解像度3次元視野の実現は，現在，苦慮しながら施行している複雑な手術手技を格段に容易とした．現時点の運用としては，各術式のなかで困難な手技だけをロボットで行う，ロボット補助下内視鏡下手術（robotically-assisted endoscopic surgery）の臨床的意義が大きいものと考えられる．

Currently, industry is focusing on developing manipulators for endoscopic surgery, and several devices have already entered clinical use. Such manipulators enhance surgeons' dexterity and improve precision of surgical techniques by filtering surgeons' tremor and scaling movements of surgical instruments. Among some clinically available robotic manipulators, an epoch-making system named “da Vinci” developed by Intuitive Surgical Inc. (Mountain View, CA, USA) is equipped with 2 articulated joints at the tip of the surgical instruments so that it has a total of 7 degrees of freedom.
