腹腔鏡下Nissen手術症例の検討

Surgical Outcome after Laparoscopic Nissen Fundoplication

　1995年3月から2000年4月までの間に当科で経験した胃食道逆流症（gastroesophageal reflux：GER）に対するNissen手術例は15例である．15例中5例で開腹手術が，ほかの10例では腹腔鏡下手術がそれぞれ施行された．腹腔鏡下手術施行例中，16歳の1例で高度の食道裂孔ヘルニアを認め，開腹への移行が行われた．両術式間の比較では腹腔鏡下手術例で手術時間が長く（p＝0.0321），在院日数が短い（p＝0.0433）という傾向を認め，出血量，周術期合併症には明らかな差を認めなかった．腹腔鏡下手術例，開腹例ともに術後経過については満足すべき状況と評価された．

Fifteen patients with gastroesophageal reflux (GER) underwent Nissen fundoplication between March 1995 and April 2001 in our department. Open procedure was performed in 5 patients and laparoscopic procedure was performed in 10.The laparoscopic operation was converted to an open procedure in a 16 year-old girl with se-vere hiatal hernia. The procedures were compared in terms of operating time, bleeding, hospital stay and post-operative complications. The operating time was significantly longer in laparoscopic operation than in open operation (p=0.0321).
