胃食道逆流症における腹腔鏡下手術

Three-port approach for laparoscopic Nissen fundoplication in children

　小児の胃食道逆流防止手術においても腹腔鏡下手術が一般的となりつつある．幼小児ではポート数が増えると器具同士が干渉し，快適な操作の妨げとなることが経験されることから，最近われわれは3ポート法を用いて腹腔鏡下Nissen手術を行っている．当施設で経験した腹腔鏡下Nissen手術は9例で，患者の年齢は平均11.8歳であった．高度裂孔ヘルニアの1例で開腹に移行し，食道炎を合併した年長例2例では4ポートで手術を施行したが，残りの6例では3ポートでの手術が可能であった．とくに合併症はなく全例で症状は改善した．術後経過は良好で再発もない．3ポート法は低侵襲で，整容性の面でも優れた術式と思われた．

From November 2004 through February 2005, 9 patients including 4 associated with esophageal hiatus hernia underwent laparoscopic Nissen fundoplication for their gastroesophageral reflux (GER) in our hospital. Age ranged from 3 months to 36 years (average, 11.8 years). Three-port and 4-port procedures were performed in 6 and 2 patients, respectively. An adult patient with esophageal hiatus hernia was converted to the open proce-dure because of severe adhesion of the hernial sac in the mediastinum.
