内視鏡下マイクロ波肝凝固療法における血管損傷の予防と対処法

　内視鏡外科手技を用いた肝臓癌に対するマイクロ波凝固療法（MCT）について，その合併症の1つである血管損傷を取り上げ，その予防と対処法について述べる．MCTによる血管損傷には出血と血管閉塞があり，これらの予防にはMCT予定部位の主要な血管の位置の把握が重要であり，それにはリアルタイムに血流を観察できるカラードップラ超音波断層装置が有用である．出血に対しては，その種類（oozing，拍動性出血，血腫，胆道内出血など）に合わせた対処が必要である．後者の血管閉塞は肝実質の萎縮や壊死を起すので，肝予備能の少ない肝硬変を合併する症例のMCTには細心の注意を払う必要がある．

Microwave coagulation therapy for liver tumor using endoscopic procedure is widely performed, and some complications have been reported. Injury to the hepatic blood vessels is one of the troublesome complications and its prevention and treatment are mentioned here. Injury to the hepatic vessels includes bleeding and occlu-sion. To prevent this injury, anatomical confirmation of the blood vessel around a liver tumor during the opera-tion is important. Color doppler ultrasonography has the high ability to detect hepatic vessels because of its easy depiction of bloodstream using color image.
