自然気胸に対する細径胸腔鏡下レーザー焼灼術の検討

Minimally Invasive Thoracoscopic Laser Ablation for Spontaneous Pneumothorax

　原発性自然気胸症例に対する細径（2〜3mm）胸腔鏡下レーザー焼灼術（以下，mini-VATS）について，胸腔鏡下肺部分切除術（以下，VATS）と比較検討した．手術時間はVATS群が66.6±22,1分に対しmini-VATS群が54.8±17.5分と短かった．術後鎮痛処置を必要とした症例はVATS群の58.8％に対し，mini-VATS群では21.2％と少なかった．術後2年での再発率は，VATS群では8.8％，mini-VATS群では7.1％と同等であった．以上より，mini-VATSはVATSより低侵襲かつ同等に有用であることが示唆された．また，自動縫合器を用いないため，医療経済的にも優れた術式であると思われる．

Is is thought to be a standard procedure for spontaneous pneumothorax to resect bullas with thoracoscopy (VATS). We have performed laser ablation with needle thoracoscopy (2-3 mm in diameter) as less invasive surgery. In this article, we investigated the usefullness of this minimally invasive procedure (mini-VATS) for spontaneous pneumothorax. In the mini-VATS group, operation time was shorter than that of VATS group (54.8±17.5 minutes vs. 66.6±22.1 minutes; p<0.05). Usage of non - steroidal anti - inflammatory drugs for postperative wound pain could be reduced in mini-VATS group (21.2% vs. 58.8%; p<0.01).
