早期胃癌に対する腹腔鏡下手術—手術成績からみた利点と問題点

Postoperative Appraisal of Laparoscopic Intra-Gastric Surgery (L.I.G.S.) for Early Gastric Cancer

　早期胃癌に対する腹腔鏡下手術にはいくつかの方法があるが，われわれが独自に考案した腹腔鏡下胃内手術は，きわめて低侵襲で有用な術式である．最近3年問で高齢者早期胃癌24例を含む30例に本法を施行した．術中合併症として注意しなければならないのは，出血と穿孔である．術後合併症では大きな問題はなかったが，2例に局所再発がみられた．いずれもsm癌症例で，内視鏡的レーザー焼灼により経過観察中である．一方，m癌症例では全例再発は認められていない．したがって，本法はリンパ節郭清を要しない早期のm癌に対してはきわめて有用であり，今後はその適応を高齢者に限ることなく拡大できるのではないかと考えている．

In the last three years since 1993, we have performed our original laparoscopic operation, namely, Laparoscopic Intra-gastric Surgery (L.I.G.S.) and its modification on patients with early gastric cancer. The indications for L.I.G.S. have been limited so far to the early staged gastric cancer in elder patients. In this series, a total of 30 patients, mainly with early gastric cancer, have been operated on by L.I.G.S.. A couple of patients were converted to mini-open surgery because of intra-operative complication, such as bleeding or minor perforation.
