乳腺切除術—吊り上げ法による内視鏡併用乳腺部分切除術

Video-assisted Partial Mastectomy using the Lesion-lifting Method

　比較的小さい乳癌に対して，美容的な観点から内視鏡を併用した乳腺部分切除を開発してきた．当初，腋窩の小切開からの吊り上げ法による乳腺部分切除術を施行したが，症例によっては手術操作が困難で，長時間を要することが多かった．そこで乳輪周囲の半周切開創から部分切除を行う手術を，主に内側腫瘍に対して施行した．腋窩リンパ節は2cmの切開から，原則としてセンチネルリンパ節生検のみとし，術中転移が判明した場合は腋窩創を延長して郭清をしている．本手術法は，腫瘍の位置によらず短い時間で施行が可能で，またディスボーザブル製品を多く消費しないので有用な方法であると考えている．

We have been developing transaxillary endoscopic partial mastectomy (TAEPM) using the lesion-lifting method since 1995, and have been getting good cosmetic results in patients with comparatively small cancer of the breast. However, long operation times are in case, with a tumor located in the inner quadrants of the breast. Thus, we changed the approaching method from the axillar to the periareolar-circular incision to shorten the time. Sentinel lymph nodes were picked up through another 2 cm-long incision in the axilla.
