Hand-assisted laparoscopic surgery（HALS）下に膵体尾部切除術を施行した膵粘液性嚢胞性腫瘍の1例

A case of mucinous cystic tumor of the pancreas successfully resected by hand-assised laparoscopic distal pancreatectomy

　患者は58歳，女性．1988年に直腸癌によるMiles手術の既往があり，当院の外科に通院中であった．1994年10月の腹部CTで膵尾部に約2cm大の嚢胞性病変を指摘されていた．2001年になって増大傾向を認めて入院となった．諸検査によって腫瘍は8cm大の膵尾部嚢胞性腫瘍と診断され，hand-assisted laparoscopic surgery（HALS）下に膵体尾部切除術を施行した．手術時間は162分，出血量は40ml以下であった．術後経過は良好で，術後15日目に退院となった．腫瘍は8×7×6cmで，病理組織学的診断はmucinous cystadenomaであった．HALS下膵体尾部切除術は比較的大きな腫瘍に対し，開腹術を選択する前に一考に値する術式であると考えられた．

A 58-year-old woman who underwent Miles operation for rectal cancer in 1988 was diagnosed as having a cystic tumor of the pancreas in October, 1994. Abdominal computed tomography (CT) revealed a tumor meas-uring 2 cm in diameter, located at the tail of the pancreas. In 2001, the patient was admitted for close examina-tion and treatment because the tumor size increased to 8 cm in diameter.
