吊り上げ法による腹腔鏡下虫垂切除術の工夫

Laparoscopic appendectomy using an abdominal wall lifting method

　吊り上げ法にて腹腔鏡下虫垂切除術を施行したので報告する．症例は22例で，平均年齢は27.9歳であった．皮下鋼線を右下腹部に2か所刺入し，吊り上げ法を施行した．臍部（5mm，scope port），右側腹部（5mm），恥骨上部（11mm）の3孔式で行った．逆行性に虫垂切除術を施行し，手術時間は平均71.5分であった．病理学的には，カタル性3例，蜂窩織炎性12例，壊疽性7例であった．術後平均入院期間は7.1日であった．術中に膀胱損傷を1例認めたが，術後合併症はなかった．腹腔鏡下胆嚢摘出術で用いられている吊り上げ法の利点は，虫垂切除術でも有効で，さらなる安全性・経済性を高めることが可能であった．

We performed laparoscopic appendectomy using an abdominal wall lifting method on 22 patients. Three ports were inserted on the abdomen; under the umbilical portion (5 mm in a diameter, scope port), right upper abdo-men (5 mm) and above left pubic joint (11 mm). Two pieces of Kirschner wire were obliquely inserted and lifted up to obtain the working space. After the diagnosis of appendicitis, the root of appendix was ligated with 2-0 or 3-0 silk by extracorporeal knot-tying and transected.
