 [Objective]To investigate the incidence of subcutaneous emphysema after robotic gastrectomy and to report our efforts to reduce the incidence of emphysema. [Methods]We retrospectively examined the incidence of subcutaneous emphysema and the effect of our approaches on reducing the incidence of emphysema in 74 cases of robot-assisted gastrectomy at our hospital from June 2019 to December 2021. The modifications included(1)low insufflation pressure, (2)shortening the port diameter and port insertion through wound edge protection, and(3)adjusting the port position. [Results]Although Subcutaneous emphysema occurred in 33 cases(45%), no major adverse events were observed. 1)Subcutaneous emphysema decreased from 28 cases(56%)to 5 cases(21%)before and after the approaches. 2)Female(p<0.05), BMI<22kg/m2(p<0.05), and no intervention(p<0.05)were identified as risk factors for subcutaneous emphysema. The multivariate analysis showed that female and BMI<22kg/m2, and no intervention(odds ratio : 5.71, 95%confidence interval : 1.71-22.73)were independent risk factors. [Summary]The procedure after robotic gastrectomy contributes to reducing the incidence of emphysema.
