　肺小結節陰影に対する胸腔鏡下手術において，術前にCTガイドに穿刺針を留置するCTガイドマーキング法（以下，マーキング）の有用性はすでに報告されている．今回，その有用性を客観的に評価する目的で，20mm以下の肺小結節36病変に対して行った31回の胸腔鏡下手術例をマーキング施行群15例と非施行群16例に分け，手術時間，ミニ開胸の有無より比較，検討した．結果は両者間で手術時間に差は認めなかったが，マーキング施行群がミニ開胸の造設頻度が低かった．また，マーキング施行群において問題となる合併症もなく，CTガイドマーキング法は肺小結節病変に対する胸腔鏡下手術において有用と思われた．

Purpose : In this study, We evaluated the usefullness of pre-operative percutaneous needle localization with Computed Tomography (CT) guidance in cases of thoracoscopic pulmonary resection of small pulmonary nodules. Method : For 36 pulmonary nodules less than 20 mm in size, 31 sessions of thoracoscopic pulmonary resection were performed. At operation, pulmonary nodules were detected using CT guide marking technique in 15 cases (marking group), and by direct digital or instrumental palpationalone in 16 cases (non marking group).
