脾機能亢進症に対する手技の工夫

Laparoscopic Splenectomy for Patients with Hypersplenism

　門脈圧亢進症合併患者に対する腹腔鏡下手術の適応は，現時点においてはまだ慎重に考慮すべきであるが，脾機能亢進症患者に及ぼす腹腔鏡下脾摘術のメリットは大きく，われわれは積極的に取り組んでいる．手術適応は，血小板数3×10⁴／μl以下または5×10⁴／μl以下で出血傾向を伴う症例である．他臓器の治療を安全に行うために，同時または二期的に手術を行うのもよい適応と考える．手術は，自動縫合器，ハーモニック・スカルペル^＊，モルセレーターなどの新しい器具を用いることで，巨脾症例であっても比較的安全に確実に施行できるようになった．

Recently the indication for laparoscopic splenectomy has been extended to the patients with hypers-plenism. One hundred consecutive patients underwent laparoscopic splenectomy in Kyushu University Hospital. Fifty one of 100 patients had hypersplenism. The operation was successfully performed by using new technologies such as autosuture device, Harmonic scalpel, morcellator and so on. In conclusion laparos-copic splenectomy is a feasible and safe method for patients with hypersplenism.
