われわれの吊り上げ式後腹膜腔鏡視下脊椎手術の適応と問題点

Problems and the Clinical Significance of Gasless Retroperitoneoscopic Spine Surgery

　CO₂ガスを用いない吊り上げ式後腹膜腔鏡視下脊椎手術は，最小侵襲手術手技として不安定椎の固定や椎間板切除に有用な手技となった．内視鏡技術およびガスを使用しない吊り上げ式器械とビデオ技術の進歩とあいまって，後腹膜腔の手術視野は急速に良好になってきている．吊り上げ式の利点は，気腹法の生理的血行動態に与える合併症が避けられること，脊椎手術では欠かせない持続吸引が可能なことである．今後さらなる技術の進歩と器械の開発により，実用性ある手技として発展していくものと思われる．

Retroperitoneoscopy without CO₂ insufflation permitted disc removal and fusion of unstable spines in a minimally invasive way. Technical progress, gasless abdominal wall lifting apparatus, and videoendoscopic equipment markedly improved the visual and working space in the retroperitoneum. The advantages of gasless abdominal wall lifting and retroperitoneoscopic spine surgery are that physiological and hemodynamic complications of CO₂ insufflation can be avoided and that can be used, continuous suction which is necessary for spine surgery.
