乳癌に対する内視鏡下手術

Endoscopic Treatment for Breast Cancer

　近年，乳房温存療法が普及し，縮小手術による治療が確立しつつある．そこでわれわれは，内視鏡下乳腺手術を開発した．本術式は吊り上げ法を用いた内視鏡補助下乳腺部分切除術と気腋窩（8mmHgCO₂）下に行う内視鏡下腋窩リンパ節郭清術により構成され，術中はすべてハーモニック・スカルペルを使用し結紮は不要であった．ハーモニック・スカルペルと気腋窩の安全性については，豚による実験で確認した．現在まで11例の患者に本法を施行しているが合併症はなく，平均手術時間158分，出血量32〜250g，郭清リンパ節個数5〜22個であり，乳腺部分切除に伴うLevel　Ⅱまでの郭清では十分な視野が得られ，有用な方法と考えられた．

The breast conservation surgery is becoming a routine procedure in the clinical fields. The metastasis of axillary lymph node is important as prognostic factor but over 5 cm incision is necessary for axillary lymph node dissection. So we applied endoscopic treatment for breast cancer. Our procedures consist of endoscope assisted mastectomy (partial or total) by using lifting system and axillary lymph node dissection by using balloon dissection method under pneumo-axilla. Coagulating scissors (Harmonic scalpel, Ultracision corp.) was used in these procedures so that ligation was not necessary.
