内視鏡下乳房温存手術において切除範囲が乳腺の1／3を超える場合に，吊り上げ法による広背筋脂肪弁充填同時再建法を行っているので紹介する．本術式において，体位を半側臥位とすることにより，手術台の回転のみで仰臥位と側臥位を確保することで手術時間を短縮しえた．また，スピードトラックによる上肢吊り上げにより上肢を任意の角度に固定することが可能になった．現在までに施行した内視鏡下乳房温存手術51例のうち33例（64.7％）に広背筋脂肪弁による同時再建を行った．観察期間の中央値は20か月と短いが，乳房内の再発は1例もなく，根治性と整容性の両面で優れた術式と考えられる．
In endoscopy assisted surgery for breast cancer, when more than one third of the breast must be resected, we perform reconstruction using the latissimus dorsi. The patient is fixed in the lateral position, and the table is rotated when a supine position is required. Lifting the arm using the speed track. we can secure any angle of the arm. We performed endoscopy assisted breast conserving surgery for 51 patients with breast cancer. Of these. 33 patients were performed with immediate reconstruction using the latissimus dorsi. There was no local recurrence at the 20 months median follow up periods.
