卵巣嚢腫に対する腹腔鏡下手術

　腹腔鏡下手術を安全・確実に行うためには，装置類や器具・器材の整備は当然であるが，症例の厳正な選択と疾患に応じた術式の選択肢があることや，状況に応じた柔軟な手技・手法で対応することも大切である．良性の嚢胞性の卵巣嚢腫に対する腹腔鏡下手術の方法においても，大きく分けて体腔内法（intra-corporeal method：ICM）と体腔外法（extra-corporeal method：ECM）がある．種々の条件の下に卵巣嚢腫の手術を安全・確実に遂行するためにも，症例と術者の技量に合った術式の選択を行い，その状況に応じた手技・手法で柔軟に手術展開をはかることが必要である．本稿では，主に癒着を伴うチョコレート嚢腫に対するICMとECMとともに，われわれが開発したSAND Balloon Catheter^®の使用法も紹介する．

　In order to conduct laparoscopic surgery safely and correctly, the instruments and materials should be in good condition. It is also important to carefully select patients suitable for such operations, to have several appropriate selections for each disease, and to be technically flexible depending on the coditions encountered. Laparoscopic ovarian cystectomy for benign ovarian cysts can he performed by the intra-corporeal method (ICM) or entra-corporeal method (ECM).
