卵巣嚢腫摘出術—体内法と体外法

Laparoscopic ovarian cystectomy:intracorporeal and extracorporeal techniques

　卵巣嚢腫は産婦人科疾患のなかでも比較的頻度が高く，腹部救急疾患として他科で腹腔鏡下に対処される場合もある．術前検査でのUSGやMRI画像と腫瘍マーカー値などから良性の卵巣嚢腫と判断されれば，大半が腹腔鏡下手術の対象となる．しかし，小児や生殖年齢の女性に対しては妊孕能温存（妊娠可能な状態）を，妊娠を伴っている場合には妊娠継続を心がけた手術内容が求められる．手術の選択肢としては，腹腔鏡による体内法，体外法，混合法，必要な場合には開腹手術への移行となる．まず，腹腔鏡下に腹腔内の状態を確認したうえで手術の可否を判断し，少しでも低侵襲で有効な手法を選択し提供することが大切と考える．本稿では，腹腔鏡が整備されておれば比較的簡便にできる体外法の器具類と手術手順を中心に，また体内での縫合操作を要する体内法やfinger assist technique，留意点などについても紹介する．

Laparoscopic surgery for ovarian cyst is one of the most commonly performed operations. Although it is (generally) performed by a gynecologist, in emergency it is often handled by a general surgeon. Preoperative work-up like USG, MRI and tumor markers are used to determine whether the tumor is malignant or benign. If the tumor is diagnosed as benign, then it is considered a candidate for laparoscopic surgery. In children and women of reproductive age, the preservation of fertility is essential and in pregnant women, preventing miscar-riage and immature labor is the primary concern.
