産婦人科領域における超音波凝固切開装置の応用

Use of an Ultrasonic Coagulating Dissector in the Field of Obstetrics and Gynecology

　内視鏡下手術では，手技や術式の違いや使用する器機や器具の違いでその手術内容は大きく異なる．特に，腹腔鏡下手術における最近の周辺器機や器具の目覚ましい進歩により，これまでの手術概念が大きく覆されようとしている．その1つに，超音波メス（超音波凝固切開装置）による手術展開が挙げられる．特に標的血管を把持して圧迫しておくことで凝固と切離の血管処理が可能となり，腹腔鏡下手術における複雑で厄介な作業をより簡便で確実なものとした．本稿では，婦人科領域における代表的な手術内容である癒着剥離，子宮摘出，子宮筋腫摘出，悪性疾患，造腟術などでの超音波メスの使用を紹介しながら，本器の有利な点やピットフォールなどについても触れる．

Endoscopic surgery varies considerably depending on the operative technique and also on the surgerical instruments and apparatus used. In particular, established surgical concepts are being overturned by the recent notable progress in instruments and apparatus for laparoscopic surgery. One such advance is the development of an ultrasonic knife (ultrasonic coagulating dissector) . With this knife, target blood vessel is grasped firmly, and it can be coagulated and dissected. This has made the comlicated procedures of laparoscopic surgery simpler and more certain.
