腹腔鏡下卵巣嚢腫摘出術の臨床研究—特に体外手術法を中心として

Laparoscopic Ovarian Cystectomy.

　良性の嚢胞性の卵巣嚢腫に対して，簡便性と安全性ならびに確実性を求めた“体外手術法”という新たな手術手技の概念を導入して4年が経過した．その間に行った268症例の卵巣腫瘍に対する手術的アプローチ方法の内訳は，腹腔鏡による“体外手術法”158例，同じく体内手術法24例などの190症例（71％）である．腹腔鏡下手術以外の方法としてはmini-laparotomyが14例（5％）で，開腹手術が64例（24％）である．“体外手術法”で行った158例のうち140例（89％）が成功したが，残る18例は開腹手術への移行を必要とした．しかし，1994年末までの体外手術法による手術成功率は87％（97／112）であったのが，手術の習熟，術式の改善・改良，器具類の開発などにより，現在では93％（43／46）にまで向上した．本法は，少ない侵襲で開腹手術と同じ内容の手術が簡便に，容易に，かつ確実にできる方法である．

Laparoscope assisted extra-corporeal cystectomy for benign ovarian cyst is a simple and safe procedure. Four years have already been passed since the introduction of new technique in our institution. The subjects consisted of 268 cases with ovarian tumors. Among them, 190 cases with ovarian cysyts were treated with laparo-scopic assistance. Mini-laparotomy was performed for 14 cases with ovarian cysts and open surgery was performed in 64 cases with ovarian solid tumors.
