The COVID-19 pandemic shed new light on the significance of intensive care worldwide. Appropriate delivery of intensive care as part of a medical system was a critical factor to save patients in relation to the overwhelming demand of severely ill patients with COVID-19 infections. We must continue to discuss the following issues:1-What is an intensive care unit (ICU)? 2-Who is an intensivist?, 3-Where and how should intensivists be utilized?, 4-Who will use the ICU and decide on criteria for admissions/discharge?, 5- What is appropriate reimbursement?, 6-What does the future hold in the context of a national health care plan for the future with an aging society and a decrease in the number of medical personnel? The author proposes some introductory comments.
