Intensive Care Units (ICU) are an essential function for acute care hospitals with considerable impact on quality of care and hospital finances. The premium medical fees (i.e. ICU administrative fees 1 and 2) were newly established upon revision of the medical fee table in 2014. The higher fee got attention from hospitals, but most hospitals are not able to meet the space requirement, which should be more than 20㎡ per bed. Therefore, the higher medical fee does not necessarily promote better quality of care in the ICU.

 Appropriate deployment of high-quality ICU care regardless of region is an important issue to provide better critical care for society. It is necessary to evaluate the level of health care services provided in the ICU using valid tools, such as SOFA in terms of medical fees. Furthermore, consolidation of hospitals with ICUs would be considered under the circumstance that the productive-age population and healthcare budget are constrained in the future.
