Oxygen therapy for patients with an acute myocardial infarction (AMI) has been used for more than a half century. To justify this common practice, a significant number of laboratory and clinical studies have examined the benefits and harms of oxygen therapy on systemic and coronary hemodynamic status and clinical outcomes. However, a clear benefit of its routine use for patients with an acute AMI has not been confirmed. Recently, several randomized controlled trials and systematic reviews have not shown benefit from the liberal use of oxygen for patients with an AMI or acute coronary syndrome (ACS). Recent guidelines from Western countries recommend that oxygen therapy should not be given to patients with ACS without low arterial oxygen saturation (SaO2). Therefore, administration of oxygen therapy based on SaO2 is reasonable for these patients.
