副甲状腺嚢胞の1症例

A Case of Parathyroid Cyst

I．はじめに

　1905年Goris^1）が最初に副甲状腺嚢胞について記載してから欧米では169例，本邦では38例の報告がある。しかし副甲状腺嚢胞に関する報告のほとんどが外科施設における症例であり耳鼻咽喉科医にはあまり知られていない。

　一方頭頸部外科の概念が耳鼻咽喉科に適用されてきた現在，頸部腫瘤を主訴として耳鼻咽喉科を受診する症例が増加している。今回われわれは嚢胞内貯留液の副甲状腺ホルモン測定により副甲状腺嚢胞疑いの診断のもとに手術を行い，Maxwellらの診断基準^11）に基づき副甲状腺嚢胞と診断した症例を経験したので報告する。

　Surgical treatment of a parathyroid cyst was first recorded by Goris in 1905. To date, 168 cases have been reported in the literature, and 37 cases in Japan.

　But these cases have been mostly published from the department of surgery, therefore the otolaryngologist haven't known the parathyroid cyst. In recent years, patients who have been complaining of palpable neck tumors tend to call on otolaryngologists.

　In a patient with a cystic mass in the neck, the level of parathyroid hormone and thyroid hormone in the cystic fluid were measured.

　The level of parathyroid hormone in the cystic fluid was higher than that in peripheral blood but the level of thyroid hormone in the cystic fluid was lower than that in peripheral blood.

　From the fact mentioned above, parathyroid cyst was diagnosed. The preparative diagnosis was compatible with postoperative pathological diagnosis according to the criteria of Maxwell et al.
