　はじめに

　前頸部の腫瘤を訴え来院する患者に副甲状腺の異常を認めることは少ない。非機能性副甲状腺嚢胞（non functioning parathyroid cyst：NFPC）は欧文誌において200例余り^1,2），本邦においても80例余りが報告されているにすぎず，比較的稀な疾患である。NFPCはその存在部位と臨床症状，画像所見から甲状腺嚢胞として手術されやすいが^3,4），鑑別疾患の1つとして念頭におけば術前に診断することは可能とされる^{1〜3,5〜7）}。今回われわれは，甲状腺腫瘍として紹介され，手術によりNFPCと判明した症例を経験したので，その診断および治療について文献的考察を加え報告する。

A 23-year-old woman presented with a thyroid tumor. Physical examination and computed tomo-graphy revealed a soft, round mass measuring 3.5 cm in diameter just below the left lobe of the thy-roid gland. Labolatory examinations revealed thatthyroid hormones and serum calcium were normal. Fine needle aspiration biopsy yielded colorless watery fluid containing scanty foam cells. Ultra-sonograhy showed a thin-walled cyst. Surgical removal was couducted, and the cyst was easily dissected from surrounding stractures. Intact PTH in the excised cyst was extremely high (24,700 pg/ ml. Histopathologic studies revealed normal para-thyroid cell in fibrous wall relevant to parathyroid cyst.
