不明熱と診断された小児急性上顎洞炎の2症例

Acute Maxillary Sinusitis of Children

　I．はじめに

　発熱の原因を探る上で，耳鼻咽喉科領域の精査が必要なことは今や常識である。特に小児の不明熱の際に，急性中耳炎の有無を調べることは忘れてはならないこととされている。簡便な耳鏡で小児科医がそれを行うことも日常化している。しかし，逆に，耳の精査が強調されるあまり，他の部位，特に鼻副鼻腔は，頬部腫脹や高度の鼻漏がないかぎり注意されることはまれであり，ひとつの盲点となっている。最近われわれは，小児科で不明熱として入院治療を受けていた急性上顎洞炎の2症例を経験した。これらの症例を報告するとともに，小児の急性副鼻腔炎について検討を行った。

　Two cases of acute maxillary sinusitis in children were reported.

　They were initially treated for fever of undetermined origin. The reason for delay in diagnosis was that they had few local signs in spite of their severe generalized symptoms including high fever, general malaise and loss of apetite. The diagnosis was finally established by x-ray examination. We emphasized that the possibility of acute maxillary sinusitis in children who have fever of undetermined origin should be kept in mind.
