鼻性脳膿瘍の2症例

Intracranial Complication of the Frontal Sinus Infection.

　I．はじめに

　耳鼻咽喉科領域における頭蓋内合併症は，最近少なくなってきており，とくに鼻性のものは耳性のものに比べて報告はきわめて少ない。しかし，その死亡率は現在でも高く，とくに抗生物質の使用で症状が隠されることがあり，その診断が遅れることが多い。最近，筆者らは前頭洞病変から生じたと思われる鼻性頭蓋内合併症を2例経験した。1例は剖検により脳膿瘍，化膿性髄膜炎が明らかにされたものであり，1例はCTで硬膜下膿瘍が発見され，適切な治療を行うことができ治癒した症例である。この2例の概要を報告し，鼻性頭蓋内合併症の診断，感染経路などにつき若干の文献的考察を加えた。

　Two cases of intracranial complication due tofrontal sinus infection are reported.

　Case 1, a 66-year-old male had a brain abscess and meningitis secondary to frontal sinus infection. These intracranial complications were discovered by autopsy.

　Case 2, a 71-year-old female had subdural empyema. CT scan was very useful in rapid diagnosis of the intracranial complication. Craniotomy and Killian's operation were done and she was discharged without neurologic sequelae.
