内耳障害と顔面神経麻痺を併発した—外耳道ムカデ異物

A CASE OF CENTIPEDE BITE IN THE EXTERNAL AUDITORY MEATUS

　I．はじめに

　外耳道異物のなかでも有生異物の報告は少なく，とくに有生異物に神経症状を併発した症例の報告はきわめて稀である^1）2）。最近私どもは外耳道をムカデにかまれた後に，同側の内耳障害と顔面神経麻痺を併発した症例を経験したので報告する。

　A 26-year-old male visited our clinic after being bited his left external auditory meatus by a centipede. His chief complaints were otalgia, sensorineural hearing loss, tinnitus, vertigo and facial nerve paralysis on the left side. Otoneurological examinations revealed sensory epithelial damage of the inner ear and geniculate ganglion lesion along the facial nerve.

　The patient was treated by steroids, ATP, etc. After one year follow-up, there were marked improvements of subjective equilibrium disturbance and facial nerve paralysis, but there were no changes in sensorineural hearing loss and tinnitus. Discussions were made on the etiology of the inner ear lesion and the facial nerve paraly-sis.
