外傷性外耳道閉鎖症—4症例の検討と手術法の選択

POST-TRAUMATIC ATRESIA OF THE EXTERNAL AUDITORY CANAL

　後天性外耳道閉鎖症は比較的稀な疾患で，原因疾患として外傷，火傷，慢性中耳炎，慢性外耳道湿疹などに続発して生ずることがある。いつたん狭窄，閉鎖したものでは，その修復形成手術は部位診断や手術手技の難しさ，さらに術後再狭窄と意外と難しく，その成績は好ましくないことが多い^1）〜3）。

　私どもは外傷性，ことに交通事故による頭部外傷例，耳後部の刺切創症例，鼓室形成術後の症例の各例に発生した外耳道閉鎖症を経験した。4症例の報告とこれらの症例の診療を通じて本症発生の予防ならびに治療上の問題とくに手術法の選択について考察を加えた。

　The development of post-traumatic atresia of the external auditory canal may be due to various causes. The cases reported here were due to (1) head injury from traffic accident (contrecoup);(2) chronic otitis media with added tympanoplasty ; (3) head injury with temporal bone fracture in a traffic accident ; and, (4) penetrating wound of the retroauricular region from a crashed glass. The authors emphasized the necessity of early appropriate preventive management of the ear canal by otologist in case of head injury accompanied with a bleeding ear and with cases of chronic erosion of the canal in a circumscribed form. The treatment as well as observations in theses should be continued for at least the period of 3 weeks.
