 A new surgical technique for the avoidance of the stenosis of the tracheostome devised by Kurt Jatho was introduced. By using this technique concomitant with the laryngectomy the insertion of the tracheal canula is unnecessary from immediately after the operation and, therefore, any kind of stomal trouble can be excluded. The high points of this surgical technique are as follows.

 1. A tracheostomy for the anesthesia tube is performed at the level of third and fourth tracheal ring for the preservation of upper trachealrings.

 2. An U type skin incision for the laryngectomy is preferable.

 3. After the closure of any defect in the hypopharynx and/or esophagus the lower one third of the internal caput of the sternocleidomastoid muscle is excised bilaterally for the convenience of the following procedure.

 4. The first and second tracheal ring just above the superior edge of the stoma is incised at the midline and retracted laterally by two sutures on the lateral surface of trachea through subperiosteum of the clavicle to the upper lateral breast for the fixation.

 The tracheal edges are approximated to the skin edge starting at the lower midline.

 5. The lower part of the U shaped skin flap is excised for the moderate upward tension of the tracheostomy.
