中耳根治手術に於ける一次的植皮と聴力恢復とについて

　中耳根治手術の主要な目的は，すべての感染巣を完全に除去し，これによつて中耳を乾燥せしめるにある。然るに手術後省耳漏の認められることが多い。又根治手術後に於て完全に術創の上皮化するために，数ヵ月を要するということは，患者にとつては経済的にも，身体的にも負担である。従つて術後耳漏の出現が少い方法，或は治療法を短縮し得る方法は，患者のみでなく，医師にとつても益することが大きい。

　その故に手術方法の改良進歩があり，特に錐体内病変の除去に向つて努力が払われ，手術後に於ける植皮ということが次第に行われている。こゝに私の記す目的は，手術直後に行う一次的植皮が未だ余り行われていないという現状に対し，この一次的植皮の意義と，私の方法による聴力恢復という点について述べんとするのである。

Goto states that, when primary split-thickness skin graft is employed to cover the cavity crea-ted by radical mastoidectomy by endaural app-roach an improvement in hearing is permissible. Such an improvement in hearing is unattainable with skin grafts under usual methods that were heretofore performed. The author reports 150 cases of mattoidectomies performed under the new method and says that the graft after its taking serves the purpose of tympanic menbr-ane. He lists measures of precaution that are to be heeded during the procedure and diucsses methods for the necessary after-care.
