頸椎骨折に合併した外傷性椎骨動脈断裂の1症例

Traumatic Rupture of the Vertebral Artery Associated with Fracture of the Cervical Spine: A case report

I．はじめに

　閉鎖性血管外傷のうちで椎骨動脈損傷は非常に稀なものである．一般的に，閉鎖性椎骨動脈損傷では内膜剥離，intramural hemorrhageなどによる血管閉塞，動脈瘤形成，動静脈痩形成が多く^11），血管断裂を呈した症例は少なくわずか14例が報告されているに過ぎない．そのうち，死亡前に診断のついたものは2例のみであった^6,7）．

　われわれは，閉鎖性外傷によって第6，第7頸椎骨折と同部位の椎骨動脈が断裂し，頸部の皮下血腫を形成した症例を経験した．対側椎骨動脈の血行が良好であったために断裂部を結紮手術することによって根治しえたので報告する．

We report here a case of traumatic rupture of thevertebral artery associated with fracture of the cervical spine.

A 72-year-old woman was found at the bottom of the stairs by her family. She was immediately transfered to our hospital. She was, in spite of being in a state of shock, alert and oriented with normal respiration. A curvilinear laceration at the right suboccipital region was noticed. On neurological examination, she was di-agnosed as having tetraparesis. Roentogenograms con-firmed fractures of a vertebral body at C 6 and a trans-verse process at C 7.
