I．はじめに

　高齢者で高血圧症の既往があり，CT scanで被殻出血を認めた症例は高血圧性被殻出血と診断し，神経学的重症度と血腫量によって方針を決定することが一般的である^4）．

　今回著者らは1986年1月から1988年12月までの3年間に当科に入院し高血圧性被殻出血と診断した107例のうち神経学的重症度がIII以上の17症例に対して開頭血腫除去術を行った．このうち3例で術中に大脳基底核部に異常血管網を認めた．これら3症例の臨床像，放射線学的所見および病理学的所見について検討したので報告する．

Three cases of histologically proven cryptic arterio-venous (AV) malformation of the basal ganglia are re-ported.

Seventeen cases of patients showing putaminal hemorrhage in the CT scan underwent craniotomy for evacuation of a hematoma between January 1986 and December 1988. The preoperative diagnosis was hypertensive putaminal hemorrhage. In all three cases a network of abnormal vessels around the internal cap-sule was detected at surgery.
