Ⅰ．はじめに

　椎骨脳底動脈循環障害の原因を頸部にもとめると，環軸偏位^2,28），頸部の過伸展，屈曲，回転による椎骨動脈の圧迫^23,25），頸部脊椎症のためosteophyteによる椎骨動脈への圧迫^{1,8,24,26,27）}，さらに中枢側でPowers症候群，頸肋など^15,19）のように椎骨動脈起始部付近における圧迫など，その走行中，種々のレベルでの障害が報告されている．

　また外傷による類椎の骨折脱臼を伴う椎骨脳底動脈循環障害は臨床的にしばしば遭遇する病態である．動脈硬化性変化の存在はこれらの原因による循環障害を促進させる要因でもある^27）．

Brain stem and cerebellar infarction syndrome following traumatic occlusion of the vertebral artery has been described with cervical fracture-dislocation, cervical manipulation, cervical spondylosis, scalenus anticus syndrome and Powers' syndrome. Hyperextension and/or excessive rotation of the neck beyond physiological limits cause the obstruction of the vertebral artery with or without cervical fracture-dislocation. However, physiological neck rotation rarely causes the occlusion of the vertebral artery.
