ウイルスと脱髄性疾患

Virus and demyelinating disease.

はじめに

　ウイルスと脱髄性疾患との関わりあいが最初に注目されたのは，Louis Pasteurにより始められた狂犬病ワクチン接種の副作用としての脱髄で，ワクチンの副作用による脱髄の研究には100年の歴史がある。1933年，Riversらにより，これが宿主の免疫機構を介する自己免疫反応によることが示唆されたが，その後，種痘後脳炎，麻疹後脳炎など，ワクチン接種，ウイルスの全身感染に随伴する一群の脱髄性疾患についても，その発症機転への自己免疫反応の関与が明らかにされている。

　一方，ウイルスの髄鞘への直接的侵襲，つまり髄鞘支持細胞であるoligodendrocyteのウイルス感染の結果として起こる脱髄の例としては，1965年ZuReinらによって脱髄病巣にpapovavirusの存在の証明された，Progressive Multifocal Leukoencephalopathy（PML）がある。

Immune mediated myelin destruction and thedemyelination secondary to viral infection of the oligodendroglia are well accepted mechanisms of virus induced demyelination. Demyelination in the CNS may occur as a consequence of systemic viral infections and vaccinations where the host immune mechanism plays a decisive role and the evidence of direct virus invasion into the CNS is often lacking. Progressive multifocal leukoencephalopathy is the only human demyelinating disease in which demyelination is directly related to the viral infection of myelin supporting oligodendroglia. In addition, causative role of viral infections has been postulated in a variety of human demyelinating diseases, including multiple sclerosis, but no convincing evidence has been provided.
