ウイルス性神経疾患とその治療

On the treatment of viral infections in the CNS.

はじめに

　このシンポジウムのテーマとしてウイルス性神経疾患の治療を取り上げたのは，今年，1986年が脳のウイルス性疾患の治療の歴史のうえで大きな節目に当る年であるからである。つまり，Louis Pasteurがフランス科学アカデミーで自分が恐犬病の治療に成功したと宣言したのが，1886年3月1日のことで，今年はそれからちょうど100年目に当るわけである。ウイルス性疾患の治療については既に1798年にJennerが天然痘の治療法として種痘が有効であることを報告しているし，ワクチンによる感染症の治療しては，Pasteurとその協力者たちが，炭疸病をワクチンにより予防しうることを1881年に報告していた。しかし，ヒトのウイルス性神経疾患を治療しうることを証明したのはこの狂犬病の治療が最初である。これに先立つこと4年，1882年にPasteurは狂犬病の研究に着手したといわれている。

　Pasteurの業績は第一に，狂犬病の病原体は脳内接種を反復してもその病原性を維持しうることを証明したことであり，ついで病原体を動物の脳で植え継ぐことにより，病原性の安定した弱毒株を得ることに成功したことである。この後，Pasteurはこのようにして得られた固定株に感染した動物の神経組織を乾燥することにより無毒なワクチンを作ることに成功したが，この辺の状況については，同時代の日本人微生物学者志賀潔先生がその著，「細菌学を創ったひとびと」に詳述されておられる^1）

　On March 1, 1886, Louis Pasteur concluded his monumental speech at the Academy of Sci-ences in Paris by proclaiming that la prophylaxie de la rage apres morsure est fondèe. An era of vaccine treatment for viral infections in the CNS thus began. What Pasteur left for us was, indeed, great. Without vaccine, we might not be able to control the rabies endemic in Japan during the post-World War period. And fortunately, our land has been free from rabies for the past 25 years. Vaccination also made a great contribution to the management of Japanese encephalitis in Japan. In 1924, the number of patients exceeded 7, 000 and a mortality rate was more than 60% contrasting the number of reported cases being 31 in 1983. Not only for these acute infections but also for slow virus infections, vaccination appears to be effective. After mass vaccination for measles, a significant reduction in the occurrence of subacute sclerosing panencephalitis has been noted in both the U.S.A. and Japan. Thus, the first century of immunotherapy has been a glorious history of conquering of human diseases. New biotechnological tehniques now allows the production of more potent and safer vaccines.
