インターフェロン療法中にパーキンソン症状を呈した腎細胞癌

A Case of Renal Cell Carcinoma occuring Parkinsonian Symptoms during Interferon Therapy

　症例は63歳，女性。主訴は構音障害。全身倦怠感があり，施行されたCTで右腎に径5cmの充実性腫瘤を認め，右腎摘除術施行。腎細胞癌と診断された。23病日目より術後補助療法としてα型IFNを600万単位連日5日間筋注，29病日目に錐体外路症状が出現した。患者は10年来，脳循環不全のためフルナリジン塩酸塩（flunarizin：FZ）を内服継続していた。FZ内服のみ中止し，上記症状は消失した。パーキンソン症状はフルナリジン塩酸塩とIFNの相乗作用によるものと考えられた。

A 63-year-old woman, who had taken flunarizinebecause of cerebrovascular insufficiency syndrome forten years, entered our hospital because of generalfatigue. Computed tomography scan (CT) showed solidright renal mass 5 cm in diameter and radical nephrec-tomy was performed. Parkinsonian symptoms occurredafter interferon therapy (IFN-α) on the 19th post opera- tive day. None of focal neurological findings were seenprior to interferon therapy. CT scans of the brain dis-closed no abnormalities. The symptoms disappearedwith discontinuance of flunarizine.
