インターフェロンによる潰瘍性大腸炎類似の急性腸炎

Ulcerative Colitis-like Acute Colitis Caused by Interferon α and γ

　症例は47歳，男性。2年前に左腎細胞癌のため根治的腎摘除術を受けた。その後，右鎖骨上窩および縦隔リンパ節転移が出現し，1996年8月よりインターフェロン（IFN）α，γの併用療法を施行した。1997年8月より腹痛，水様性下痢が出現したため大腸内視鏡検査を施行したところ，潰瘍性大腸炎類似の急性腸炎の所見であった。IFNを中止すると症状は改善したため，IFNによる薬剤性急性腸炎と考えられた。IFNによる副作用として，本症の発生にも注意すべきと思われた。

The patient was a 47-year-old man who received rightradical nephrectomy for renal cell carcinoma two yearsago. He was admitted to our hospital with right supraclav-icular and mediastinal lymph nodes metastases, and receivedthe combined therapy of interferon-αand-γ in August1996. He tolerated the therapy well until August 1997 whenhe first noted abdominal pain and watery diarrhea. Colono-scopic examination revealed diffuse red-yellowish edema-tous mucosa from the rectum to ascending colon. Discon-tinuance of the IFN-αand-γ therapy improved hissymptoms, suggesting that IFN-αand γ might havecaused the disorder.
