前部尿道穿孔および膀胱穿孔をきたした尿道異物

Urethral Perforation and Vesical Rupture Resulted by a Vesico-Urethral Foreign Body:A Case Report

　57歳，独身男性。自慰目的にて尿道内に木製マドラーを挿入中に自己抜去困難となり当科を受診した。異物は尿道内にとどまり前部尿道を穿孔しており，経尿道的な抜去は困難と考え，経腹手術にて異物を除去した。異物は膀胱を穿孔していたが，腹膜内への穿孔はなかった。同時に膀胱瘻造設を行った。本邦において膀胱尿道異物の報告は多いが，前部尿道および膀胱穿孔を同時にきたした症例は自験例が1例目と考えられた。

A 57-year old male was admitted to our hospital, withcomplaints of urethral pain followed by insertion of awooden stick into his urethra by himself for the purposeof masturbation. But he could not remove the stick. Onphysical examination, his scrotal skin was protruded andswollen by the foreign body, which penetrated anteriorurethra and bladder wall as well. Open surgical removalof the foreign body and cystostomy was performed. At14 days postoperatively, cystostomy was removed andurination was smooth. Many reports on vesico-urethralforeign body has been made, but very few with urethralperforation and vesical rupture.
