尿路感染が誘因と考えられた頸椎硬膜外膿瘍

A CASE OF EPIDURAL ABSCESS INDUCED BY URINARY TRACT INFECTION

　尿管ステント留置中に発熱と下肢の痺れが現われ，MRIで頸椎硬膜外膿瘍が指摘された。尿路感染が誘因と考えられた。脊椎前方固定術を行い歩行器歩行は可能となったが，四肢の痺れが残存した。硬膜外膿瘍は神経根痛，背部痛，発熱などを初発症状とし，急速に神経麻痺を来すので，早期発見と早期治療が要求される疾患である。

A 54-year-old man developed fever and paralysis of lower extremities following ureteral stent indwelled for bilateral hydronephrosis for prolonged periods of time. MRI revealed epidural abscess and UTI associated with ureteral stent was suspected as probable cause. Anterior cervical interbody fusion was performed. But, paralysis was not improved. Early diagnosis is necessary before paralysis occurs.
