陰嚢内腫瘍と鑑別困難であった表皮嚢腫

EPIDERMAL CYST DEVELOPING LIKE AN INTRASCROTAL TUMOR: A CASE REPORT

　49歳，男性。左陰嚢部の腫瘍は精巣と同一エコーレベルであり，当初，陰嚢内腫瘍が疑われていた。腫瘤は会陰部にも触知した。エコー，MRIより左陰嚢皮膚と会陰部皮膚から同時に発生した2つの嚢腫と考え，陰嚢から会陰部にかけての切開にて腫瘤摘除術を行った。両腫瘤とも一部皮膚と強固に癒着しており，皮膚を合併切除した。組織学的診断はいずれも表皮嚢腫であった。

A 49-year-old man presented with a painless tumor in the left scrotum and perineal region. The tumors were adhered to the skin, but did not involve the left testis, epididymis or spermatic cord. Echo imaging revealed a cystic mass which had same echo level as a testis. Magnetic resonance imaging was the most useful modality for the diagnosis. Complete excisions were performed and the lesions were histologically diagnosed as epidermal cysts.
