原発性尿管小細胞癌の1例

Small Cell Carcinoma of the Ureter:A Case Report

　症例は86歳，男性。肉眼的血尿，頻尿と残尿感を主訴に当院を受診した。精査の結果，右下部尿管腫瘍の診断で右腎尿管摘出術を施行した。高齢であることより尿管は腫瘍の約3cm下で結紮・切断した。病理の結果は小細胞癌で切除断端は陰性であったが，粘膜から壁外へ高度の浸潤を示していた。術後後療法を行わず退院としたが術後3か月で局所再発し，疼痛を伴うため放射線照射を行った。腫瘍の縮少傾向はみられたが，術後6か月目に癌死した。

A 86-year-old man visited our hospital with the chief complaint of gross hematuria, pollakisuria and residual urine sensation. Retrograde pyelography and ureteroscopic examination revealed a tumor in the right lower ureter. Right nephroureterectomy was performed and ureter was resected at about 3 cm below the tumor because of his high age. Histological examination revealed small cell carcinoma. Histologically, surgical margin of the ureter was negative, but the tumor invaded the adventitia of the ureter.
