孤立性対側副腎転移を伴った右腎細胞癌

Renal Cell Carcinoma with Synchronous Contralateral Adrenal Metastasis:A Case Report

　45歳，男性。血尿を主訴に当科を初診し，腹部CTにて右腎に65×77mmおよび左副腎に35mmの腫瘍を認めた。静脈サンプリングでは副腎ホルモンの過剰放出は認めず，画像上他部位への転移を認めなかった。孤立性対側副腎転移を伴った右腎腫瘍の診断で，手術を施行した。病理結果は右腎細胞癌および左副腎転移であり，追加療法としてインターフェロンを投与中であるが，術後14か月の現在再発，転移を認めていない。

A 45-year-old man was admitted to our hospital with a complaint of gross hematuria. Abdominal computerized tomography revealed a 65×77mm right renal tumor and a35×35mm tumor with hypervascularity in the left adrenal gland. Hormonal examination did not show any abnormal findings. We diagnosed left adrenal mass as a metastasis from a right renal tumor, and performed right nephrectomy, preserving the right adrenal gland but removing the left adrenal gland. The pathological findings of both the resected tumors indicated renal cell carcinoma.
