　30歳，男性。左陰嚢内容の硬結を主訴に当科を初診した。Β-hCGは2,000ng/m/と異常高値を示し，画像上，多発性肺，肝転移，および後腹膜リンパ節転移を認めた。精巣の病理所見は奇形腫の成分のみであった。進行性精巣腫瘍としてPVeBV療法3コースを施行後，末梢血幹細胞移植術併用大量化学療法2コースを引き続き施行した。残存病変の切除にて生存癌細胞の残存を認めなかった。術後14か月現在再発を認めていない。

We reported the experience of high-dose chemotherapy (HDC) combined with peripheral stem cell transplantation in a 30 year-old man with advanced nonseminoma ac-companied with multiple lung metastases, multiple liver metastases and retroperitoneal lymph node metastasis.The initial level of β-hCG was 2,000ng/ml.We performed 3 courses of chemotherapy (PVeBV regimen) and peripheral stem cells were collected by apheresis on day 18 of the second course.After 3 courses of chemotherapy,β-hCG fell to 2.4ng/ml and metastatic lesions still persisted on CT scan.Therefore we performed 2 courses of HDC.
