腎動脈瘤を有したドナー腎の移植

RENAL TRANSPLANTATION USING THE KIDNEY WITH ARTERIAL ANEURYSM

　腎提供者は47歳の母親で，息子への腎移植を希望し当科入院した。動脈撮影で右腎動脈第一分枝に1.0×1.5cmの不完全な石灰化を有する嚢状動脈瘤を認めた。手術は右腎を摘出しCollins液で灌流後シャーベット状生食で単純冷却した。動脈瘤は腎体外手術により摘出し瘤末梢動脈を主幹動脈に端側吻合した。腎移植後経過は良好で，吻合部狭窄などの異常所見を認めない。

A 47 year-old female visited our institution for renal transplantation to her son. Aortography showed a right renal artery aneurysm measured 1. 0 × 1. 5 cm in diameter with incomplete calcification, which was saccular in type and located at the first bifurcation of the renal artery. Before extracorporeal arterial reconstruction the removed kidney was flushed with Collins solution and then was submerged in ice slush saline to maintain hypothermia. Under these condition aneurysmectomy was performed and then the distal branch was anastomosed to the main renal artery (end-to-side).
